 Registration for CYSS Membership
To obtain a CYSS membership: Registration is currently free under the Army Family Covenant.
1. Print and fill out the following forms.
- SNAP Tool # 1 (Required by all patrons.)
Special Needs children require medical documentation prior to registering with Child, Youth and School Services. Allergies are also considered a special need.
2. Bring.
- All forms completed.
- Identification (Proof of Eligibility).
- Child's birth certificate.
- Child's social security card.
- Child's current immunization record for children age 0 - 4th grade.
- LES/pay stubs for you and your spouse.
- Three (3) Local Emergency contacts other than you or your spouse.

Currently at Fort Bragg, we have a waiting list for all children, 6 weeks to 5 years of age at our Child Development Centers (CDC).

 Waiting List Procedures
1. Complete and submit initial waiting list application - Waiting List Form 2. Fill out the waiting list renewal form (now online!) every 90 days in order to remain active on the waiting list. Failure to renew will result in removal from waiting list. - Waiting List Renewal Form
The information compiled from these forms is used to collect sponsor information for CYSS programs and to place sponsor on waiting list for program services.

When a slot is offered, you then enroll your child in the program.
Placing your Child
- Central Enrollment will contact the sponsor and offer the first viable care option which becomes available.
- Sponsors have 24 hours to accept or decline the child care space. Once space is accepted, failure to enroll child in allotted time will result in removal from waiting list.
- Once a space is offered, a sponsor may delay the start date for their child for two weeks.
- Sponsors who turn down care that meets their preference will be removed from the waiting list unless there are extenuating circumstances.
You must obtain a CYSS membership.
Sole/dual military must submit a copy of their family care plan, DA Form 5305-R at registration or within 30 days after child's start date. If not submitted by due date, the child will not be allowed to participate in the program.

2. Health Assessment/Sports Physical
- If the child wishes to participate in any sport, the form must be signed by a licensed health care professional and returned to central enrollment prior to the child's participation. If not submitted by due date, the child will not be allowed to participate in the program.
